Composition of Dimexide

The concentrate for preparing Dimexide solution contains at least 99% dimethyl sulfoxide (DMSO) undiluted.

COMPARISON OF THE EFFECTIVENESS OF DIMEXIDE GEL AND PLACEBO IN THE TREATMENT OF PATIENTS WITH VERTEBROGENIC DORSALGIA

Relevance. According to the results of a report by the World Health Organization (WHO) in Geneva (2003), pain in the spine “has reached epidemic proportions” [11, pp. 50-55]. The frequency of pain in the spine leading to disruption of daily activities and forcing people to seek medical help reaches 80% worldwide [1, pp. 6-10, 5, pp. 51-55]. In Russia, the frequency of dorsopathies is 71-80% among all diseases of the peripheral nervous system [6, P. 24].

Currently, the prevalence rates of vertebroneurological pathology tend to increase [3, pp. 11-16.]

With timely treatment, the pain syndrome is relieved within 1-2 weeks, and in 20%, chronic disease is observed [7, pp. 590-593]. In addition, the peak incidence occurs at working age 30-55 years. [5, p. 86]. This determines the economic importance of vertebrogenic diseases, which is associated with increasing direct costs of treating patients, as well as indirect costs due to temporary disability [12, P. 496-500].

The main goals of treatment are relief of pain, neurological symptoms, and prevention of chronicity of the disease. Priority is given to conservative therapy.

The effectiveness of NSAIDs in relieving pain has been repeatedly confirmed in randomized controlled trials [2, pp. 11-16, 5. pp. 51-55]. This is due to their pronounced anti-inflammatory and analgesic effect. However, there is a wide range of adverse events associated with NSAID treatment.

Every year, NSAID-induced gastropathy leads to 100-200 thousand hospitalizations and 10-20 thousand deaths [8, pp. 47-50]. Much attention is paid to cardiovascular adverse events of NSAIDs, in particular myocardial infarction and cerebrovascular accidents [10, pp. 71-78]. Prevention, and especially treatment of class-specific complications, represents one of the most important iatrogenic problems in medicine.

Therefore, before starting therapy, it is necessary to weigh the ratio of the expected benefit and the possible danger of complications in each specific case, especially if back pain is not accompanied by other symptoms that could indicate a disease of the spine or involvement of the structures of the nervous system in the process. All of the above creates the prerequisites for finding new ways to effectively treat vertebrogenic pain. One of the possibilities to solve this problem is the use of local means.

A fundamentally important feature of their use is their predominantly local effect, despite the fact that only a small amount of the drug can enter the bloodstream. Accordingly, the systemic effects of their use, including negative ones, turn out to be minimal [9, pp. 85-89].

Purpose of the study: comparative assessment of the effectiveness of a local drug (dimexide gel) with placebo in patients with vertebrogenic dorsalgia.

Study design. On the basis of the neurological department of the MRCH, a clinical study was conducted on 68 patients aged 18 to 77 years with acute, subacute and chronic (in the acute stage) course of vertebrogenic dorsalgia, including those caused by herniated intervertebral discs, which do not require surgical treatment, confirmed by neuroimaging. Also, inclusion criteria were moderate or severe pain syndrome (score on a visual analogue scale of more than 3 points); absence of clinically significant dysfunction of the liver and gastrointestinal tract, as well as the patient’s consent to participate in the study.

Patients with tumor and infectious-inflammatory vertebrogenic diseases, concomitant neurological pathology, gastric or duodenal ulcers in the acute stage, with mental and unstable somatic diseases, as well as those who had absolute indications for surgical intervention or consequences of surgical intervention were excluded from the study. interventions on the spine.

The patients were randomly divided into 2 groups. 38 people of the first group received dimexide gel 25%, the second group of patients, 30 people, used placebo. Patients applied the drugs to the skin in the painful area 2 times a day, lightly rubbing.

Local drugs were prescribed in complex therapy (muscle relaxants, vascular therapy, B vitamins, antidepressants, massage, exercise therapy, IRT).

The total duration of treatment was 10 days. Patients were examined initially (before the start of treatment) and at the end of the course of therapy. A visual analogue scale (VAS) was used to determine the patient’s subjective sensation of pain at the time of the study and to determine the dynamics of pain intensity under the influence of therapy. To assess the severity of muscle syndrome, the muscle syndrome index (MSI) was calculated. The degree of impairment of quality of life was assessed using the Roland-Morris questionnaire. The severity of movement limitation in the affected part of the spine was also determined in degrees.

Results and discussion . In all respondents, the duration of the disease was at least 1 year, and the duration of pain before treatment was 18.4±5.3 days. The neurological status of all patients before the start of therapy was determined by pain on palpation of the spinous processes, paravertebral points of the affected part of the spine and muscle tension.

The intensity of pain according to VAS before treatment in the studied groups did not differ significantly and was 7.46 ± 0.76 points (group 1, receiving dimexide gel), 7.13 ± 1.02 points (group 2, receiving placebo). After treatment, the pain intensity in the first group was 2.58±0.59 points according to VAS, and was significantly less than in patients of group 2 - 4.73±1.13 points (p<0.05).

To study quality of life (QOL), the Roland-Morris Low Back Pain and Disability Questionnaire was used. The questionnaire contains 18 statements describing limitations due to back pain. Patients noted those that corresponded to their condition at the time of filling out the questionnaire. Then the total number of points noted by the patient was calculated (maximum - 18). Disturbances in vital functions were considered severe if the patient noted more than 7 points. QoL changes depending on the patient’s condition, which allows monitoring the treatment.

A study of quality of life before treatment showed that patients had significant limitations in daily functioning. The indicators of patients in the two groups did not differ significantly from each other. In the first group, those treated with dimexide gel, the average value of disability was 10.31 ± 1.05, and in the group receiving placebo it was 10.4 ± 1.03. After treatment, a significant improvement in quality of life was noted in the first group of patients. Thus, the average score of the questionnaire after treatment was 4.05±0.49, which indicates an improvement in the quality of life of patients in the first group by 60% compared to the initial level. In patients of group 2, when analyzing QOL, a less pronounced effect was noted - the overall indicator was 6.73 ± 1.12. , and the improvement was only 35% of the original. Thus, the results obtained once again prove the effectiveness of dimexide gel in comparison with placebo.

The data on IMI in the groups before the study corresponded to the following values: in the first 9.28±0.72 points and in the second 9.69±1.07 points. There were no significant differences between the groups.

After therapy with dimexide gel, a mild degree of UTI was observed in 29 people (76.3%), in patients receiving placebo, a mild degree of severity was detected in 10 people (33.3%). Only one patient from the first group (2.63%) had severe IMS after treatment, and when using placebo, there was no positive effect on muscle syndrome in 4 people (13.3%). The results obtained indicate the advantage of dimexide gel in relief of muscle syndrome before placebo.

Before treatment, all examined patients were found to have limited movement of the affected part of the spine due to pain. During treatment with dimexide gel, mobility limitation significantly (p<0.05) disappeared in 15 patients (39%), and no patients with identified mobility limitation were identified. After treatment in the second group of patients, the restriction disappeared only in 8 patients (27%) and in 6 people (20%) a pronounced limitation of mobility remained. It can be concluded that dimexide gel is effective in increasing the mobility of the affected part of the spine and, consequently, in improving the quality of life of patients.

Conclusion. Due to its high prevalence and significant treatment costs, back pain is not only a socially but also an economically significant problem. Due to the increase in patients with this pathology, the development of new methods of therapy remains relevant, which would increase the effectiveness of treatment with a minimum number of unwanted and dangerous effects. The use of local dosage forms may be a solution to this problem in a number of patients.

In our study, dimexide gel showed a good analgesic and anti-inflammatory effect. This drug reduced pain and muscle syndrome, improved the functional state of the spine, which was accompanied by an improved quality of life. This allows us to recommend it as an effective remedy for relieving vertebrogenic pain syndrome.
