How is a sciatic nerve block performed?

Drug blockade for neuralgia is an emergency way to relieve pain when injections, tablets and physical therapy methods fail. A blockade of the sciatic nerve for pain is carried out by injecting individually selected drugs into the affected area. Administration of drugs can be perineural or periarticular (into nervous or periarticular tissues).

There are several ways to carry out a blockade for sciatica:

  • lying on your stomach (Yasenetsky method);
  • lateral access (lying on your back);
  • lying on your side according to Moore's method;
  • anterior approach - rarely performed. There is a high probability of damage to large vessels.

As an example, we will give a simple blockade technique using the Moore method.

  1. The patient lies on his side, legs bent at the knees. The heel of the diseased limb is pressed tightly against the healthy leg.
  2. The doctor palpates the tubercle on the hip bone (trochanter) and draws a line from it to the iliac pelvic bone.
  3. A perpendicular line is drawn from the middle of the line to the coccyx.
  4. The point of drug administration is the end of the line.
  5. Within 1 minute, only up to 10% of the drug is administered.
  6. When the patient feels numbness, the needle is moved deeper without any pain and the remaining medication is injected.

If the blockade is carried out by an experienced doctor and all rules are followed, the procedure is safe. Complications are rare.

  1. If the medicine enters the vessel during administration, the patient may experience agitation, signs of dizziness and nausea, and tinnitus.
  2. If a needle damages a vessel, a bruise may remain.
  3. Failure to comply with the rules of antisepsis and asepsis is manifested by a suppurative process at the injection site.
  4. In the absence of allergy tests for the drug, allergic reactions may develop.
  5. Damage to a nerve by a needle causes the development of neurological disorders (impaired motor functions and sensitivity).
  6. The introduction of a large amount of the drug provokes compression of the nerve fibers, which will lead to even greater pain.

Before choosing a clinic and a doctor, take the time to read reviews from real patients and doctors.

Drugs for blockade

Before the doctor prescribes a blockade in the area of the piriformis muscle, it is necessary to diagnose and clarify the indications:

  • Aching pain in the gluteal region;
  • Pain when trying to cross your legs;
  • Inflammation and swelling in the piriformis muscle area.

Several drugs are used simultaneously for blockade:

  • Local anesthetics – lidocaine, novocaine.
  • Hormonal drugs - diprospan, dexamethasone, kenalog, hydrocortisone.
  • Homeopathic remedies – Tzel-T, Traumeel-S.
  • B vitamins.

It is worth noting that after administering an injection for blockade, a positive effect is observed after 15-20 minutes. Acute pain and swelling go away, muscles relax, and blood circulation normalizes.

You can read reviews about piriformis blockade and blockade for muscle pain on our website.

To relieve spasm of the piriformis muscle, use a “medicinal cocktail” consisting of at least 2-3 drugs. This simultaneously solves several problems - rapid pain relief, muscle relaxation (spasm is eliminated) and reduction of inflammation.

Causes of pinched sciatic nerve

The nature of the development of pathology and clinical manifestations are very diverse. The most common cause of the disease is compression of nerve fibers in the structure of the spine. Pressure on the nerve roots sometimes leads to their inflammation, accompanied by severe pain. Symptoms manifest themselves differently, depending on the location of the pathological process.

  1. When one of the roots of the sacral nerve plexuses is compressed by a hernial protrusion, the symptoms manifest as sharp pain, intense when coughing and movement. On the first day, signs of impaired skin sensitivity and muscle weakness appear.
  2. A root pinched by a dense ligament or bone growths, displacement of discs or narrowing of the spinal canal caused by osteochondrosis causes gradually increasing pain and signs of complete or partial paresthesia. Muscle weakness appears later, and if the process is advanced, muscle atrophy cannot be ruled out.
  3. Compression of the sciatic nerve by muscles (called tunnel syndrome) can be a consequence of articular pathologies, spinal diseases, injuries, flat feet, and unsuccessful injections in the buttock. It manifests itself as pain in the buttock, burning pain in the sole area, loss of control of the functions of the foot.
  4. Cut and puncture wounds, injuries and bruises of the sciatic nerve are accompanied by moderate pain. By blocking impulse transmission, they can provoke muscle atrophy, the development of paralysis, dry skin and ulcers on the legs.

A kind of trigger for the development of pain can be:

  • the influence of unusual physical activity in patients with a tendency to physical inactivity;
  • prolonged stay in a forced position at work;
  • vibration or power loads;
  • body hypothermia;
  • infectious pathologies.

The greatest vulnerability to the development of sciatica is observed in middle-aged and elderly patients. Aging of the body leads to degenerative changes in the intervertebral discs. They become coarser, gradually lose moisture, elasticity and height, which leads to compression of the nerve roots and the development of acute pain.

Causes of pathology

The disease develops when the sciatic nerve is pinched, injured or inflamed.

A variety of factors can cause this situation:

  • injury to the back area;
  • radicular syndrome;
  • chronic diseases of the spine (osteochondrosis, intervertebral hernia, arthrosis, osteoporosis, spondylitis, scoliosis, protrusion);
  • benign or malignant neoplasms in the spinal column;
  • infections affecting nerve trunks;
  • inflammation of the back muscles;
  • inflammatory processes in the blood vessels of the sacrum;
  • pathologies of the hip joint;
  • diseases of the abdominal organs;
  • prolonged sitting;
  • unsuccessful injections into the epidural cavity;
  • postoperative complications.

They increase the risk of developing the disease and provoke relapses in the chronic form:

  • hypothermia of the body;
  • prolonged stay in an uncomfortable position;
  • long work at the computer;
  • improper performance of exercises during sports;
  • increased physical activity;
  • constant work in the garden (sometimes lumboischialgia is called summer residents’ disease);
  • poor posture;
  • age-related degenerative processes in the spine;
  • excess weight;
  • frequent stressful situations;
  • depression;
  • pregnancy;
  • alcohol abuse;
  • smoking.

Symptoms of the disease

The main symptom of lumboischialgia is acute lumbago (sharp pain of a shooting nature) in the buttocks and back of the legs, radiating to the lower extremities. The pain is provoked by sudden movements, lifting heavy objects, sneezing, coughing. Attacks can last several minutes or several days.

The patient may experience other symptoms:

  • decreased or loss of sensitivity in the skin of the buttocks and affected leg;
  • stiffness in the lower back;
  • muscle tension;
  • pain when standing on your leg;
  • cramps in the calf muscle;
  • numbness of the toes;
  • pallor of the skin in the sacral area;
  • increased body temperature;
  • chills and fever;
  • weakness in the legs;
  • indigestion;
  • difficulty and pain in urination and defecation;
  • itching, burning or subcutaneous throbbing.

If left untreated, blood circulation is impaired. Over time, lumbago with sciatica can cause urinary and fecal incontinence, sexual dysfunction, cause a decrease in muscle mass in one limb, and lead to disability.

Possible complications

If you want to minimize the risk of complications from conduction anesthesia
in Moscow, contact the CELT clinic. Our doctors have performed this procedure many times and know how to avoid them. It is customary to distinguish the following types of complications:

  • toxic - arise as a result of incorrect selection of dosage and concentration of the drug or its penetration into the vascular bed, characterized by various symptoms: from dizziness to falling into a coma;
  • inflammatory - appear as a result of improper procedure, use of an inappropriate or expired drug, and are characterized by the development of periostitis, meningitis;
  • allergic - characterized by various symptoms, ranging from itching and drop in blood pressure to difficulty breathing and cardiac arrest;
  • traumatic - consist of damage to blood vessels, nerves, pleural or abdominal cavity during a procedure with corresponding symptoms.
