In the past few years, scientists have been attracted to the beneficial properties of fats obtained from marine fish and mammals.

Fish oils are rich in Omega-3 polyunsaturated fatty acids (PUFAs), mainly eicosapentaenoic acid and docosahexaenoic acid. A large number of clinical observations around the world have established the positive effect of Omega-3 PUFAs on the composition of blood lipids, on the synthesis of eicosanoids (prostaglandins, thromboxanes, leukotrienes), which regulate blood clotting processes, vascular tone, immune status of the body, etc.

Another useful component of fish oil also deserves great attention - alkylglycerides. Alkylglycerides are glycerolipids with an alkyl bond. Alkylglycerides were first isolated by Swedish physician Astrid Brohult, who used bone marrow to treat children with leukemia. When analyzing the composition of bone marrow, she proved that its immunomodulatory effect is associated precisely with the presence of alkylglycerides. Alkylglycerides are contained in mother's milk (about 0.1%), providing protection to the child until his own immune system is fully formed. Small amounts of alkylglycerides are found in products of animal origin (cow's milk, meat, liver, spleen). But the richest alkylglycerides turned out to be marine inhabitants - fish and mammals. The largest amount of alkylglycerides is found in shark liver - about 30%. Shark liver oil has been known for about 40 years as an effective preventive and therapeutic agent.

As a result of experimental studies, it was established that alkylglycerides have a general strengthening, immunocorrective effect, help reduce blood cholesterol levels, improve vision, and the functional state of the musculoskeletal system. Shark liver oil also contains vitamins A, E, D, squalene, and squalamine. Squalene is an intermediate product of cholesterol metabolism. Consumption of squalene promotes the excretion of cholesterol through the gastrointestinal tract, which leads to a decrease in the level of total cholesterol and low-density lipoprotein cholesterol in the blood. Squalene also has pronounced anti-inflammatory and antioxidant effects. It captures oxygen and saturates tissues with it, thereby preventing the destructive effects of free radicals on cells.

40 patients with coronary artery disease, hypertension stages I-II were observed. with GLP. All patients received a hyposodium antiatherogenic diet during the 3-week study period.

A single-center, open-label, randomized controlled trial was conducted. The patients were divided into two representative groups of 20 people: the main group (6 men and 14 women, average age 56.5 years) and the comparison group (7 men and 13 women, average age 58.5 years). During the same period of time, patients in the main group received a dietary supplement containing shark liver oil, 1 capsule 3 times a day.

During the study, shark liver oil was well tolerated; no allergic reactions, dyspepsia or other intolerance phenomena were noted. As a result of diet therapy with the inclusion of shark liver oil, the level of systolic and diastolic blood pressure significantly decreased.

Analysis of the results of biochemical studies in blood serum showed that the inclusion of shark liver oil in the diet contributes to a statistically significant decrease in the level of total cholesterol and LDL cholesterol, the concentration of triglycerides tends to decrease.

In the light of modern ideas about the determining role of cytokines in immune reactions, the study of cytokine status was of particular relevance. Interleukin 1b is a pro-inflammatory cytokine that plays a leading role in the development of cardiovascular pathology, in particular atherosclerosis. A statistically significant decrease in it in patients of the main group (by 46%) indicates the effectiveness of using shark liver oil. In the comparison group, its level decreased by only 4%.

The data obtained indicate the immunomodulatory effect of alkylglycerides in shark liver oil. Their effect is to reduce the severity of disorders of immune status indicators that accompany the course of the underlying disease. Particularly important is the fact that the use of dietary supplements does not affect unchanged immunological parameters and contributes to the normalization of impaired parameters, regardless of whether they were higher or lower than normal. It should be emphasized that the positive effect of dietary supplements on immunological parameters contributes to the coordinated functioning of the immune system of the whole organism.

Thus, the use of shark liver oil against the background of an antiatherogenic diet in patients with ischemic heart disease, hypertension, hypertension and obesity has a beneficial effect on the clinical picture of these diseases, and also has an immunomodulatory effect.
